Unfortunately this means that the adapter is broken. You won't be able to communicate with it with UART, or upgrade the firmware over USB. The only possible way to reprogram the adapter is to program it through the debug-SPI pins (you would need to remove the casing, solder wires on the USB programming connector), but we cannot support this for customers.
